Spin Master Recalls Aqua Dots Children Became Unconscious After Swallowing Beads

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Aqua Dots

Units: About 4.2 million

Distributor: Spin Master, of Toronto, Canada

Hazard: The coating on the beads that causes the beads to stick to each other when water is added contains a chemical that can turn toxic when many are ingested. Children who swallow the beads can become comatose, develop respiratory depression, or have seizures.

Incidents/Injuries: CPSC has received two reports over the past several days of children swallowing Aqua Dots. A 20-month-old child swallowed several dozen beads. He became dizzy and vomited several times before slipping into a comatose state for a period of time, was hospitalized, and has since fully recovered. A second child also vomited and slipped into a comatose state and was hospitalized for five days.

Description: The recalled toy is a craft kit which allows children to create various multi-dimensional designs using small colored beads. The beads fuse together when sprayed with water. The recall applies to all models of Aqua Dots. The product is available in various different kits with accessories such as a drying fan, applicator pen, design templates for the beads, and spray bottle. The product is labeled for ages 4+.

Sold at: Mass merchandisers nationwide from April 2007 through November 2007 for between $17 and $30.

Swimways Corp. Recalls "Skippy" Pool Toys Due to Laceration Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: "Skippy" Pool Toys

Units: About 31,000

Manufacturer: Swimways Corp., of Virginia Beach, Va.

Hazard: The elastic tongue of the fish can break and forcefully come out and cut the users' hands during launching of the toy.

Incidents/Injuries: Swimways has received 24 reports of the pool toy breaking during use. There were five reports of injuries to children including one requiring stitches to a child's hand. Another child's thumb nail was ripped back from the nail bed.

Description: This recall involves a rubber pool toy shaped like a fish. The fish are blue, purple or green colored and measure 8 ½ inches long by 5 inches wide. A loop of elastic tubing shaped like a tongue comes out of the mouth of the fish and is used for launching the fish across the water like a slingshot. "WARNING. Do not aim at another person. Made in China" is printed on the bottom of the fish.

Description: The recall includes plastic models of a dragster and funny car. The dragster is approximately 22 inches long, black in color with various sponsor decals on the toys. The funny car is approximately 14 inches long and is also black in color. Both models have the ISL logo imprinted on the underside.

Description: The recalled item is a pearl-like pink bead attachment on the outside packaging of the gift sets and a zipper pull/wrist band on a cosmetic lip gloss set. Serious Head Injuries Prompt Recall of Bumbo Baby Sitter Seats - New Warnings and Instructions to Be Provided To Consumers

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Bumbo 'Baby Sitter" Seats

Units: About 1 million

Manufacturer: Bumbo International, of South Africa

Hazard: If the seat is placed on a table, countertop, chair, or other elevated surface, young children can arch their backs, flip out of the Bumbo seat, and fall onto the floor, posing a risk of serious head injuries.

Incidents/Injuries: CPSC has received 28 reports of young children falling out of the Bumbo Baby Sitter seat, including three skull fractures, which occurred when children fell out of chairs that had been placed on tables.

Description: The bottom of the children's seat is round and flat with a diameter of about 15 inches. It is constructed of a single piece of molded foam and comes in yellow, blue, purple, pink, aqua, and lime green. The seat has leg holes and seat back that wraps completely around the child. On the front of the seat in raised lettering is the word 'Bumbo" with the image of an elephant on top. The bottom of the seat has the following words: 'Manufactured by Bumbo South Africa Material: Polyurethane World Patent No. PCT: ZA/1999/00030." The back of the seat contains the following 'WARNING" ñ 'Never use on a raised surface. Never use as a car seat or bath seat. Designed for floor level use only. Never leave your baby unattended as the seat is not designed to be totally restrictive and may not prevent release of your baby in the event of vigorous movement."

Sold by: Target, Wal-Mart, Sears, Toys R Us, Babies R Us, USA Babies and various other toy and children's stores nationwide, and various online sellers, from August 2003 through October 2007 for about $40.

Remedy: Consumers should never use the infant seat on a table, countertop, chair, or other elevated surface. Consumers can contact Bumbo to obtain new warning label stickers and instructions, free of charge. The new warning label will state: 'WARNING ñ Prevent Falls; Never use on any elevated surface." Consumers should use the Bumbo seat at ground level, but should never leave a child unattended.

Name of Product: Magnetic Game Pieces sold with "Cars" Themed Backpacks

Units: About 110,000

Importer: Global Design Concepts Inc., of New York, N.Y.

Hazard: Small magnets inside the four game pieces that were sold with the "Cars" backpack can fall out of their plastic enclosure. Magnets found by young children can be swallowed or aspirated. If more than one magnet is swallowed, the magnets can attract each other and cause intestinal perforation or blockage, which can be fatal.

Incidents/Injuries: The firm has received three reports of a magnet that became loose. No injuries have been reported.

Description: The 12 inch "Cars" Backpack was sold with four sealed game pieces that contain magnets. The game pieces included a red plastic car; a blue plastic car; and two red and black plastic disks that have 'GAS," 'OIL" and 'SERVICE" printed in red letters on the black perimeter of the disks. 'Warning: Choking Hazard-Small Parts. Not for children under 3 years" is printed on the backpack hangtag. 'Disney Presents a PIXAR film" is printed on the gray vinyl front of the backpack.

Payless ShoeSource Recalls Girls' Boots Due to Fall Hazard

Hazard: The guitar-shaped zipper pulls on each boot can interlock while a child is walking, causing the child to trip and fall.

Incidents/Injuries: Payless has received five reports of the zipper pulls interlocking, including one report of a four-year-old girl who fell and skinned her knees.

Description: This recall involves black jeweled girls' boots. The boots have a silhouette of a guitar on the outer leg and a guitar-shaped zipper pull on the inner leg. 'Hannah Montana" is engraved on the boot outsole and a photo of the singer and show's character is printed on the box. The boots were sold in girls' sizes 10 ®&Mac246; to 4 ®&Mac246; . No other styles or models are involved in this recall.

Description: This recall involves the Albert's Halloween skull pails filled with candy mix. The skull-shaped pail is white with green eyes, nose and teeth. The pail has a clear lid with a label in which 'Albert's" and 'Skull Pails Filled with Halloween Candy Mix" is printed. The pail measures about 5 ®&Mac246; inches high.

Description: The recalled puppet theater has red panels on the front and sides and a chalkboard signboard on top. The puppet theater measures about 24-inches in length, 6-inches in width and about 28-inches in height.

Huffy Recalls Bicycles Due To Cranks Falling Off; Riders Can Lose Control of Bike

Name of Product: 2007 Huffy 'Howler" and 'Highland" Bicycles

Units: About 22,000

Distributor: Huffy Corp., of Miamisburg, Ohio

Hazard: The bicycle crank can unexpectedly come off, causing the rider to lose control, fall and suffer serious injuries.

Incidents/Injuries: Huffy has received two reports of the crank coming off, resulting in one injury.

Description: The recall involves 2007 model year, multi-speed bicycles with 26-inch, 24-inch or 20-inch wheels. The 'Howler" was sold in black (model K3587, boy's model), blue (model K4587, men's model), and red (model K6587, men's model). The 'Highland" was sold in white (model K4597, women's model) and blue (model K6597, women's model). The name "Howler" or "Highland" is printed on the frame of the bicycle, and the name "Huffy" is on the front of the frame. Model numbers are located on a label on the bottom of the frame where the crank is attached to the bicycle.

Sold at: Kmart stores nationwide from May 2007 through July 2007 for between $80 and $100.

Hazard: If the cup is dropped, the colorful face on the cup can break off and leave small parts or sharp exposed edges that can pose a choking or laceration hazard to young children.

Incidents/Injuries: Starbucks has received seven reports of the cups breaking, including two reports in which a child began to choke on a broken piece. No injuries have been reported.

Description: The recall includes four styles of Starbucks children's plastic 10-ounce cups. Styles include: 'Dot" Red Ladybug; 'Dash" Green Turtle; Bunny Pink; and Chick Yellow cups.

Flip-Flop Alphabet Blocks The recalled wooden toys have blocks that flip with letters, words, numbers, and pictures on them. The toy measures about 10 1/8" x 10" x 2". 'First Learning" and UPC #834162001212 are printed on the products' packaging.

Sold at: KB Toys stores nationwide. The Pull-Along Wagons were sold from August 2005 to August 2007 for between $9 and $13. Bunk Beds Recalled by d-Scan Due to Collapse Hazard

Name of Product: Jubee Bunk Beds

Distributor: d-Scan Inc., of South Boston, Va., a subsidiary of Tvilum-Scanbirk, of Denmark

Hazard: The recalled bunk beds do not comply with federal safety standards and have wooden side slat supports that can separate from the bed frame causing the upper bunk to collapse.

Incidents/Injuries: The firm is aware of seven incidents in which the upper bunk collapsed after the side slat supports separated from the bunk bedÕs frame. No injuries have been reported.

Description: The twin bunk beds are designed for children and were sold in a kit to be assembled by consumers. Only model number 82008 is included in this recall. The bunk beds are made of maple wood with a four-step ladder attached to the side and three pullout drawers under the bottom bunk. These bunk beds have no markings or labeling.

Sold at: Specialty furniture stores nationwide and on various Web sites from December 2004 through April 2006 for about $600.

Manufactured in: Denmark

Remedy: Consumers who have not already received and installed a free repair kit for the side slat supports should immediately stop using the bunk beds until repairs can be made. Contact the firm to receive a free repair kit that can easily be installed at home. Retailers have contacted some consumers who purchased the bunk beds directly.

Ohalee Inc. Recalls Youth All-Terrain Vehicles Due to Mechanical Hazards; Vehicles Could Put Young Riders At Risk of Injury

Name of Product: Ohalee FA-A70 Youth ATVs

Hazard: These youth ATVs lack front brakes and a tire pressure gauge, the date of manufacture is not printed on side of the tires, and the front suspension is solid and does not allow for travel. Additionally, the flag pole bracket is not the correct size, and the handlebars do not have padding covering sharp edges. There is no storage location for the ownerÕs manual, and the manual itself does not contain complete information on the safe operation and maintenance of the ATV. These defects could result in an unsafe riding condition, posing a risk of injury to young drivers.

Description: This recall involves the Ohalee FA-A70 Youth ATVs, which has the name ÒOhaleeÓ on the right and left side of each ATV. The 70cc size ATVs were available in the following colors: red, blue, and black.

BCI Recalls Swing Sets Due to Fall Hazard

Manufacturer: BCI Burke Company LLC, of Fond du Lac, Wisc.

Hazard: The connection on the top beam of the swing set can break, causing the swing beam to collapse, posing a fall hazard to the user. A user or bystander also can be injured by the falling top rail.

Incidents/Injuries: The firm has received four reports of the swing set beam breaking and one report of child with bruises after falling from the swing.

About 1 Million Simplicity Cribs Recalled Due To Failures Resulting in Infant Deaths

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC) is announcing today a voluntary recall with Simplicity Inc., of Reading, Pa., of about 1 million cribs. The drop-side can detach from the crib, which can create a dangerous gap and lead to the entrapment and suffocation of infants. CPSC is aware of two deaths in Simplicity manufactured cribs with older style hardware, including a 9-month-old child and a 6-month-old child, where the drop-side was installed upside down. CPSC is also aware of seven infant entrapments and 55 incidents in these cribs.

CPSC is also investigating the death of a 1-year-old child in a Simplicity crib with newer style hardware, in which the drop-side was installed upside down. CPSC is warning parents and caregivers to check all Simplicity cribs to make sure the drop-side is installed right side up.

The drop-side failures result from both the hardware and crib design, which allow consumers to unintentionally install the drop-side upside down. This, in turn, can weaken the hardware and cause the drop-side to detach from the crib. When the drop-side detaches, it creates a gap in which infants can become entrapped.

CPSC is also aware of two incidents that occurred when the drop-side was correctly installed with older style hardware, though the upside down installation greatly increases the risk of failure.

The recalled Simplicity crib models include: Aspen 3 in 1, Aspen 4 in 1, Nursery-in-a-Box, Crib N Changer Combo, Chelsea and Pooh 4 in 1. The recall also involves the following Simplicity cribs that used the Graco logo: Aspen 3 in 1, Ultra 3 in 1, Ultra 4 in1, Ultra 5 in 1, Whitney and the Trio.

The recalled cribs have one of the following model numbers, which can be found on the envelope attached to the mattress support and on the label attached to the headboard: 4600, 4605, 4705, 5000, 8000, 8324, 8800, 8740, 8910, 8994, 8050, 8750, 8760, and 8996.

The cribs, which were made in China, were sold in department stores, childrenÕs stores and mass merchandisers nationwide from January 1998 through May 2007 for between $100 and $300.

As an immediate precaution, consumers should check to see if the drop-side is installed right side up. To do this, check to see that the slightly rounded rail with the decorative groove is installed at the top and the plain rail is on the bottom. Next, consumers should make sure the drop-side is securely attached to the tracks in all four corners.

If the drop-side is installed upside down or not securely attached, consumers should stop using the crib immediately. Incorrect installation can cause permanent damage to the hardware and re-installing the drop-side using the same hardware is unsafe.

Consumers should check to see if their crib contains the recalled hardware. Recalled hardware has a flexible tab at the bottom of the lower tracks and the top of the lower tracks are open. Newer hardware, which is not subject to the recall, has the flexible tab located at the top of the lower track, and a permanent stop at the bottom.


Fisher-Price Recalls Bongo Band Toys Due to Violation of Lead Paint Standard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Big Big World 6-in-1 Bongo Band toys

Units: About 8,900

Importer: Fisher-Price Inc., of East Aurora, N.Y.

Hazard: Surface paints on the toys contain excessive levels of lead, which violates the federal law prohibiting lead paint on children's toys.

Incidents/Injuries: None reported.

Description: The recalled toys have two bongos, including one with a yellow and green plastic drum base with a blue drum surface. The other bongo is yellow and green plastic drum base with an orange drum surface with ÒItÕs a Big, Big WorldÓ printed on it. The toys were sold with animal shaped accessories including a monkey, bird, tambourine and drum stick. The toys have product number K9343 inside the orange drum. A date code between 139-7SH and 232-7SH is printed on the drumÕs orange ring surface. Bongo Band toys included in this recall must have both the product number and date code.

Sold at: Retail stores nationwide from July 2007 through August 2007 for about $20.

Remedy: Consumers should immediately take the recalled toys away from children and contact Fisher-Price for instructions on returning the product in order to receive a free replacement toy.

The recalled models were manufactured between July 31, 2006 and August 20, 2007 and have a date code between 212-6CK through 325-6CK or 001-7CK through 232-7CK marked on the bottom of the product. The packaging on the Freightway Transport model is marked H5705 and the packaging on the Special Track Pack model is marked K3013.

Sold at: Retail stores nationwide from September 2006 through August 2007 for between $3 and $16.

NettoCollection Recalls Cribs Due to Entrapment and Strangulation Hazard

Name of Product: "Moderne" and "Loft" Cribs

Units: About 400

Importer/Distributor: NettoCollection LLC, of New York, N.Y.

Hazard: The crib slats can separate from the side rails, posing an entrapment and strangulation hazard to young children.

Incidents/Injuries: NettoCollection has received three reports of the side rail spindles separating from the top rail. No injuries have been reported.

Description: The crib side rails are made of wood and sold with a brown finish. Only cribs with model numbers NC-137 and NC-140 and date codes 9/03 and 02/04 are included in the recall. The model numbers, date codes and "Made in Poland" are printed on a label on the crib end panels.

Sold at: Juvenile furniture stores nationwide from October 2003 through June 2005 for about $1,350.
